Thank you for being a patient at Orthopedic Associates
The following general information is in response to frequently asked questions within our office. If you have a question not addressed by the text below, please contact us.
Initial Visit
To make your first visit as convenient as possible we ask that you print and complete the patient registration and initial visit forms that you will find on this web site. We also ask that you bring your insurance cards with you to your appointment.
Telephone Calls
The telephone is a vital link with our patients. Patients are encouraged to contact our office with any questions or concerns they may have concerning their treatment. The doctor's assistant will be happy to help you and check with the doctor about your concerns. If you need to speak with the doctor directly, a message will be taken and he will return your call as soon as possible. In all cases emergency calls will be handled promptly. We respectfully request that you limit routine calls about medication, surgeries, appointments, insurance and fees to our regular business hours.
Billing and Payments:
The fees charged in our office are customary for our locality and specialty. We appreciate and encourage payment for services at the time of your visit. Patients are encouraged to discuss elective surgery fees or any other questions about coverage with their insurance carrier.
We participate with multiple managed health care plans and insurance carriers. In order to process your claim correctly, we ask that you bring all insurance information and a photo ID with you each visit to ensure proper billing and identification. Frequently, insurance policies do not cover "everything" or "100%". There are often limitations and exclusions to coverage. If you have any questions concerning your coverage, please contact your insurance carrier.
We accept most major insurance companies and we will bill all covered services to your insurance company. Please be sure to bring your insurance information to your first visit.
Payment is expected at time of service for copays, coinsurance, deductibles and any non-covered services.
For your convenience payment can be made by cash, check, Visa, Mastercard or Discover.

Workers Compensation:
When you are being treated for a work related injury or problem, please be prepared to provide us with your date of accident, employer and insurance carrier name and address at the time of your first visit.
Disability Forms:
As a courtesy to our patients, we will complete disability forms. We ask that you allow 5 to 7 working days for completion.
